Birth Certificate is issued by Local Government and Community Development (LG&CD) in Punjab Province. It is issued by relevant Union Council. If a person submits application for birth certificate in concerned Union Council within 60 days of birth the delivery time is 3 working days. If person submits application for birth certificate after 60 days, the delivery time is 7 working days. The departmental fee for issuance of this Certificate is rupees 100. • Bank Challan Form
• Birth Certificate (Hospital Birth) or School Certificate or Attested Vaccination Card Copy (attested)
• Attested Affidavit Rs.50 (2 witnesses) in case of Birth in House(attested)
