Death Certificate in Punjab Pakistan
Local Government and Community Development (LG & CD) is the relevant department for Death Certificate. Relevant Union Council issues Death Certificate. A person can go directly to the concerned Union Council with required documents to get Death Certificate. Normally a Union Council issues death certificate in three days.
If you go to the e-Khidmat Markaz of Punjab Government you can obtain the certificate in three days after payment of 100 rupees.
Documents required getting Death Certificate
- You need an application Form which are available in Union Council and also available in the e-Khidmat Markaz (Attested)
- Death Certificate of Hospital if death in Hospital
- Affidavit if Death is in House
- Bank Challan Form
If you go to e-Khidmat Markaz of Punjab Government you only need to fill the form and pay the 100 rupee fee on bank challan form along with Hospital Death Certificate (if death in hospital) to get the Death Certificate. If you go to concerned Union Council you may need all above mentioned documents.

My deceased father in law residential address is of Mohni Road Lahore and was burried in Guru Mangat Graveyard Gulberg Lahore. Where should I go to get his Death Certificate. He died in 2014.

My father died 12years ago .this time I need his death certificate I go to union council and asked him thay checked and asked me to there is no record of death so what can I do for death certificate which is 12years late

You need to provide them the Hospital Certificate if he died in any hospital
otherwise you need to file suit in civil court for that purpose
My wife died 3 months ago, today my son went to union council for registration of death but Secretary Union Council refused being late. Is it correct?

You need to file application and tell the reason of delay and may also pay late fee for this
He can not refuse simply if it is late

My father passed away in Lahore and was buried there, whereas his CNIC has the addresses of karachi. Which Union council should I go to for the duplicate death certificate? The Lahore one or the Karachi one?
The main problem is that my father’s second wife lives in lahore and she doesnt want me to have the copy of his death certificate. I only have the copy of my fathers CNIC.
What is the procedure and how do I go about it?

First you need to check in which union council you reported the death (if you reported)
If still unreported you need to report it first and then ask for death certificate

Sir my mother died in 2005 but we didn’t get the death certificate issued. Please guide me regarding the procedure for applying. She died in a hospital but we’ve lost the hospital record too and the hospital also has long been closed now. Seems like the process of e-khidmat is not as simple as you state. E – khidmat refused saying they don’t deal with Cantt cases. The issue we face is that place of death is Hameed Latif and residence is DHA. Both cantonment boards ( Cantt and Walton) are refusing saying the place is out of jurisdiction.

My wife died due to negligence of a private doctor and his team. Oxygen cylinder became empty and supply of oxygen stopped when nobody went to the basement to replace the empty cylinder with the filled one.. Dr. shifted her to the ventilator to hide their negligence. After 7 more days, he declared death. As we agitated at their negligence, doctor hasn’t issued death certificate to us. More than 3 months have passed, we have become too late. How can I get death certificate in such condition?
You can contact with higher authorities of health department or also take legal actions against them

My uncle deceased in early 2000 however his death certificate is not issued. Now our union council is demanding RS7000 in the name of late fee. They are asking us to hire an advocate to prove death.
